Information for Clients

I’ve sent in my referral form-have you received it?
The referral HUB will send a letter to you directly to say that they have received your form when they have put it on the system. If someone helped you to complete the form then they will also be notified.
Where am I on the waiting list?
It can be very difficult to say exactly where you are on the waiting list as sometimes there are emergencies that unfortunately happen in people’s lives that may mean their case is given priority over yours.
However most of the vacancies are filled according to who has been waiting on the list the longest.
My circumstances have changed
If you feel that your situation has changed or you no longer need our services you need to let us know. Please contact us on 01803 208058 or e-mail us and a resettlement officer will be able to update your case notes and advise you accordingly.
My address or contact details need changing
Please let the Resettlement Officers know of any changes by calling 01803 208058, or by e-mailing us.

Information for Providers

I have a vacancy
Please complete the ‘Service User Notification Form’ for all vacancies which is available from the do it online panel on the right of this page The form should be completed as early as possible to enable the team to have time to fill the vacancy appropriately. The form should be returned by emailing it to the Resettlement team at the following E-mail:- referralhub@torbay.gov.uk
I have a query about an assessment I have received
Please contact the Resettlement Team on 01803 208058 if you would like to discuss a case.
I need advice about a difficult client
As above please contact the Resettlement Team who will be able to offer advice and solutions and talk about the options.
How do I let you know if I am able to accept a client or I have to refuse a client?
You will need to complete the acceptance/ refusal form which is available from the do it online panel on the right of this page and ensure that you have given a reason for your decision by ticking the appropriate box- the Resettlement Team will then look at this decision and may wish to discuss the case further.
